Road one: India
- Finish the six-year course, including the 310-day internship year and the state exams.
- Obtain proof that you are eligible for a licence in Bulgaria, as the NMC's 2021 rules require.
- Pass the FMGE, which NExT is due to replace.
- Complete the 12-month supervised internship in India and register with your state medical council.
Prepare for the FMGE from the clinical years: the exam follows the Indian syllabus, which does not match the Bulgarian one subject for subject.

Road two: Europe
A Bulgarian MD is an EU qualification. Doctors who want to work in another EU country apply to that country's authority and pass its language test. Practising in Bulgaria needs Bulgarian and registration with the Bulgarian Medical Association. For specialisation, each country runs its own selection.
